***IMPORTANT: Due to our current waitlist, if your new patient paperwork is not completed at least 2-business days before your scheduled appointment, your appointment will be rescheduled. Completing your paperwork at least 2-business days in advance allows your provider ample time to review the information and prepare for your appointment. Completed packets also serve as confirmation that you will be attending your intake appointment.
